TBI Needs Assessment Survey
The survey should be completed by individuals with a TBI and their support systems – friends, family, employers, etc. Please feel free to distribute to interested persons. All responses to the survey are confidential. No one will be contacted for follow-up. Please feel free to add any comments that will better explain needs of individuals with a TBI.
Dr. Mary Aitken, MD, MPH, an associate professor of pediatrics in the College of Medicine at the University of Arkansas for Medical Sciences and Arkansas Children’s Hospital, is assisting the organizations with the survey.
